How can I get my records?
The best and easiest option is to have your records sent directly to your new primary care provider. You can fill out forms in their office and have them faxed to us at (346) 202-0087 or use our online form to request for a records transfer. Please allow 3-5 business days for your records to be processed.
You may request a copy of your records to keep personally. Paper records will be unavailable once our office closes. To request access to your patient portal, email your request to account@ebenezerhcs.com
Records are kept for 7 years from the date of service.
Can I make and appointment?
We are no longer taking clinic appointments. Due to moving and maintenance the clinic will be unavailable to patients. We will maintain office hours in order to facilitate records process and any other clerical needs.

We mainly used DLO labs for our patient's lab need. You can get access to your labs ordered by us, as well as your other doctors here: https://www.dlolab.com/patients/patient-lab-results

Call you local county health department and ask the health department to check OSIIS. OSIIS is the Oklahoma State Immunization Information System, which is a centralized immunization registry that keeps electronic records of vaccines given by health care providers. All records administored in our clinc have been stored in OSIIS.

As a reminder, after careful consideration, we have made the difficult decision to permanently close our office at the end of this year.
Please be advised that there is only one remaining week for in-person appointments, as our office will be closed during the week of Thanksgiving. We understand the importance of continuity in your healthcare, and we are committed to providing support during this transition period.
Appointments in December will be limited to urgent cases. Consequently, our online scheduling system will be disabled. However, we will continue to manage established medications for patients seen within the past 6 months, and renewal referrals for patients seen within the past year until the office closes. For medication refills please be sure to contact your pharmacy. They can fax requests to 405-759-7022.
We strongly encourage you to establish care with a new primary care provider as soon as possible, especially for any new diagnoses or screenings you may require. To assist you in this process, we have compiled a list of alternative care providers in the area, which is included with this letter. You may also get in contact with your insurance provider for further assistance with your search.

As we transition, I want to assure you that Ebenezer Health Care Services will continue to process your prescription refills and referrals for 90 days from the date of this notice. During this period, please ensure that you have sufficient medication and that any necessary referrals are in place.
To ensure a smooth transition to a new primary care provider, we encourage you to contact your insurance company for assistance in finding a suitable replacement. They will be able to provide you with a list of in-network providers who can meet your healthcare needs. This should be done as soon as possible in order to avoid lapses in your care.
If you require copies of your medical records, please have your new primary care office submit your request via fax to 405-759-7022 or email to accounts@ebenezerhcs.com (mailto:accounts@ebenezerhcs.com). Our practice will retain your medical records for seven years from the date of your last service, ensuring that your healthcare history is preserved and accessible as needed.
